Digital Data Management

Context

CARE was developed in early 2020 and the first use case was to manage COVID patients. The tool was mostly utilized by lower-level healthcare facilities which lacked any digital infrastructure and very basic physical infrastructure that catered to the majority of the slightly/moderately ill COVID patients. But with the 10BedICU Project, the use case is shifting to managing the critically ill admitted to the ICU.

To smoothly transition to a Critical Care focussed tool, the workflows around patient management need to be amended.

Patient Flow

There are various steps in managing a patient. The most common steps are:

  1. Registration

  2. Initial Health Assessment

  3. Consultation

  4. Rounds

  5. Tests and other investigation/Diagnostics

  6. Discharge

There are also various actors (user type) involved in this process. They are

  1. Administrative staff (Staff)

  2. Nurse

  3. Doctor (this can be a doctor at the hospital or a remote specialist)

  4. Health department Officials (District Admins)

Workflows

Registration

Any new patient is identified in CARE with a combination of their registered phone number and date of birth along with their name for recognition. Therefore, the registration form needs to capture the following information:

  • Name

  • Phone number

  • Date of Birth

  • Other demographic details

The master data for the registration form is available at CARE: Patient flow: Master Data

Once a patient is registered, there is a unique patient ID generated by the system for the patient. All the subsequent information relating to that patient is logged into the same Digital File with the patient ID. Therefore all records of the patients in all the connected hospitals will be maintained and will be available to any treating doctor.

Initial Assessment

This section includes the medical history and other health details of the patient. This section needs to be filled out only at the first visit that the patient takes to any hospital, upon registration. Subsequently, this section may be updated or edited to add more information.

The section of medical history will automatically keep getting updated as the patient’s clinical details are entered into the system.
